Post-discharge follow-up for Kenyan hospitals

Care continues after the patient goes home.

PonaPath helps hospital teams keep in touch with patients after discharge. Patients get short SMS check-ins on agreed days and reply with a number. Anyone who is unwell reaches a nurse quickly.

PonaPath is preparing its first hospital pilot. It is not yet in use with patients.

How it works

Simple for patients, clear for ward teams.

Enrol at discharge

The ward nurse records the patient's SMS consent and contact details. The patient receives a discharge message with their hospital's phone number.

Check-ins by SMS

Check-ins go out on the days the hospital agrees. Patients reply with one number; caregivers can be included where the hospital chooses.

One list for the ward

Replies arrive in one list for the hospital's staff. Each has an owner and a response deadline, and every case is closed with a recorded outcome.

Patient safety

Built so that a patient in trouble is not missed.

  • Warning words in English and Kiswahili, such as "chest pain" or "nashindwa kupumua", put a reply at the top of the list with the shortest deadline.
  • A patient who reports a warning sign, or asks to be called, gets an SMS at once: go to the nearest hospital or call 999 / 112.
  • If nobody responds in time, the hospital's on-call phone is alerted, then the ward manager.
  • Patients who stop replying get one reminder; if they stay silent, a nurse is asked to phone them.
  • Patients can reply STOP at any time to stop messages.
